Who prints this
Anyone selling without a checkout: a market stall, a craft fair, a bakery taking cake orders, a phone order for something made to size. The paper copy is the only record until it is typed up.

Why it is laid out this way
Contact and delivery sit above the item lines, so they are taken while the customer is still giving details rather than after the order has been read back. Totals sit directly under the table, and postage has a line of its own because it is the number most often left out of a total added up by hand.
